📢Important information regarding post-operative wound care 📢
ℹ️ Due to recent changes with Secondary care (Hospitals) contracting Primary care (GP Surgeries), Primary care are no longer responsible for any post-operative wound care.
When you are discharged after your operation/procedure the plans for your ongoing care, i.e clips/ stitch removals, dressing changes etc should be highlighted to you in your discharge information. This will then be carried out within a hospital setting.
Please ask for your discharge letter BEFORE you leave the hospital.
If you are not given the details of follow up appointment by your discharge team then please do call the ward you were an inpatient with to ascertain the plans for your follow up care . If you are still not able to obtain the follow up care needed them please contact the surgery who will complete a referral form to Wye Valley Trust on your behalf, who will arrange this with you directly

Practice Nurses
Our nurses see patients by appointment and are responsible for much of the annual reviews and follow-up care for management of chronic illnesses such as:
- Diabetes
- Heart Disease
- Asthma & COPD
- Blood tests
- Health checks
- New patient checks
- Anti-coagulation monitoring
- Blood pressure checks
- ECGs (if requested by a GP)
- Weight checks
- Pill checks
They can give advice on a wide range of general health issues and offer the following:
- Childhood Immunisations
- Vaccinations
- Travel vaccination/advice – please book a 20 minute appointment
- Cervical smears
- Family planning- all the doctors can also give advice on family planning. Emergency contraception ('morning after pill') is always available, please contact the surgery. Coils, caps and implants can also be fitted at the surgery
- Removal of sutures and wound dressings (we do not provide post operative dressings please see below)
- HRT

The Practice is always committed to providing relevant information to people who use its services and the following methods will be used to provide this information:
- During your consultation
- Information leaflets
- Posters displayed in communal areas
- Information and health promotion boards within the patient waiting area, e.g. local advocacy services
- Practice booklet
